Health History
You are starting a journey to optimal health. This is a custom to you conversation and unique to you process. No two people are the same and therefore no two coaching journeys are. To start on a strong foundation please complete the HEALTH HISTORY FORM. It will come straight to me upon submission and allow me to really have a full in-depth understanding of your health. Combined with your application and our discovery call I am able to understand your past and present and lock in your vision for the future.

When you look at your life, your goals, your fulfillment, even your peace, it is critical to know what drives you. Put another way, what you value is in fact what is pushing or pulling you in life. When looking at achieving optimal health you want to ensure your vision aligns with who you are and what matters to you most. A very powerful tool I’ve been using for almost 20 years to support clients in understanding what’s going on behind the scenes is the What Drives Me Values Workbook. Fall in love and bookmark this tool. The balanced living workbook is something we will review every session to look at the balance you’re experiencing in life. Integrative health looks at both primary and secondary foods, and this workbook allows you to identify what primary foods might benefit from some additional attention.
